What You'll Be Doing
Patient Support & Care Coordination: Manage a panel of patients, ensuring they receive the necessary clinical and non-clinical care to manage their chronic conditions effectively.
Resource Coordination: Identify and connect patients with community resources that can enhance their care.
Patient Education: Provide guidance on chronic disease management, empowering patients with knowledge to improve their health.
Medication Management: Assist with medication reconciliation, adherence monitoring, identifying potential concerns, and coordinating refills.
Service Coordination: Ensure timely access to essential services, including Home Health, DME, Home Infusion, and other critical patient needs.
Patient Engagement: Address patient inquiries with a proactive approach, fostering an open dialogue to understand and resolve concerns.
Appointment & Referral Assistance: Support patients by managing referrals and scheduling necessary medical appointments.
What We're Looking For
Active Multi-State/Compact License (LPN) (NLC) (LVN)
Technical Skills: Proficiency in Microsoft Office Suite
Strong Communication: Excellent written and verbal communication skills
Bonus Points If You Have:
Experience with Epic electronic medical records
Bilingual proficiency
Additional single-state LPN licensures

A dedicated, HIPAA-compliant workspace
Physical Requirements
Ability to remain seated for extended periods while working on a computer
Strong verbal communication skills for telephone-based patient interactions
Ability to listen attentively and take thorough, real-time notes during calls
