Responsibilities:The Supportive Care/Palliative Care Nurse Practitioner (SPCNP) works in collaboration with HarmonyCares Medical Group staff to provide comprehensive, quality palliative care services to patients under the direction of the supervising Regional CMO. The SPCNP will be responsible for providing patient visits for palliative care patients, both new and established and coordinating their plan of care with other designated staff, especially the palliative care team. Patient visits will include visits to patient’s homes, facilities and hospitals. In addition, the SPCNP will engage in independent decision-making about a patient’s healthcare concerns and goals of care and provide palliative care services on an as needed basis. Collaborating Providers will be available for consultation, collaboration, or referral.
