Job Summary: Under physician's orders, the nurse is responsible for identifying and coordinating patient/family needs and for providing preventative and supportive care to the terminally ill patient/family unit. The nurse utilizes assessment, intervention and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit.
Job Responsibilities:
- Demonstrates the ability to obtain patient history, perform an age-specific physical, psychosocial and spiritual assessment and initiates appropriate referral, intervention and collaboration between the patient, family, Interdisciplinary team (IDT), physician and partnering facility (as applicable) following policy and practice guidelines.
- As RN Case Manager provides continuity of care, care coordination, identifies patient/family goals, develops a Plan of Care with patient, family, IDT, attending and physician, partnering facility and evaluates progress towards goals. The Plan of Care will be updated according to policy/guidelines and as needs change.
- Identifies age-specific patient and caregiver learning needs, motivations, barriers, abilities and preferences and utilize teaching strategies/instructions and follow-through to match.
- Promotes a culture of safety by implementing policy, guidelines, applying best practice safety assessment and management principles. Applies critical thinking, judgment and ethical principles for problem solving in all patient and caregiver clinical situations.
- Adheres to principles of infection control, including Standard Precautions on all patients, use appropriate isolation & PPE, perform proper hand hygiene and provide appropriate patient/family teaching.
- Prepares for and participates in collaborative dialogue, care planning and care coordination before and during the IDT meeting. S
- eeks to incorporate expertise and involvement of all disciplines in the plan of care per patient/family goals and requests.
- Documentation is compliant with all organizational standards including productivity, timeliness and adheres to relevant policies and practice guidelines.
- Following a patient’s death, assists with closure contact and support in collaboration with IDT.
- Directs and supervises the care provided by the LPN and Home Health Aide following agency policy. Evaluates services provided by the Home Health Aide every two weeks with periodic on-site supervisory visits.
- Demonstrates knowledge of Empath Health policies, guidelines and clinical procedures and pertinent State and Federal Rules and Regulations and Joint Commission standards.
- Participates in the team responsibility of providing 24/hour a day, 7 days a week patient care.
- Educates and represents the community regarding hospice care philosophy and concept through a case by case consultation with persons in the community or educational in service or seminar as assigned.
- Provides education to IDT members regarding nursing aspects of patient/family care.
- Demonstrates the ability to administer, monitor and document medications as prescribed by attending physician. Uses age-specific care practices and provide appropriate patient/family teaching.
- Attends in-service and educational programs including all mandatory meetings, trainings, and skills review.
- Develops and maintains appropriate professional relationships with patients, families, caregivers and community partners, i.e., facility staff, physicians, etc.
- Assumes personal responsibility for maintaining up to date credentials and Continuing Education requirements including BLS, licensure and TB.
- Performs other duties as assigned by Supervisor.
Required Skills & Experience:
- Minimum of one (1) year of related experience and/or training or equivalent combination of education and experience.
Preferred Skills & Experience:
- Prefer one (1) year experience in an acute or post-acute care setting.
- Preferred experience caring for terminal patients and others with life threatening illnesses.
- Ability to type 25 wpm is highly preferred.
Required Education:
Required Certifications & Licensure:
- Valid Florida license as a Registered Nurse.
- Valid Florida State Driver’s License.
- Valid BLS certification from the American Red Cross or American Heart Association.
- For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion.
- Must provide proof of valid automobile insurance and current MVR, a copy of which will be placed in the employee’s HR file.
