In this role, you’ll:
Take ownership of a dedicated home health caseload
Perform comprehensive in-home assessments addressing physical, psychosocial, and environmental needs
Create, implement, and adjust individualized plans of care
Complete start-of-care visits, routine nursing visits, and re-evaluations
Collaborate with physicians, Clinical Supervisors, and care partners to ensure seamless care
Communicate changes in patient condition and advocate for patient needs
Maintain accurate, timely documentation that supports high-quality outcomes and compliance
Active, unrestricted RN license
At least 1 year of clinical nursing experience
Home Health or Hospice experience preferred
Experience with HomeCare HomeBase strongly preferred
Associate or Bachelor’s Degree in Nursing preferred
Strong assessment skills, critical thinking, and communication
Willingness to travel within the assigned service area (up to 50%)
Reliable transportation, valid driver’s license, and current auto insurance
