This role is essential in ensuring patients experience a smooth transition into home health and hospice care. You will support the intake process, ensuring patients and families receive the guidance and coordination needed for a smooth admission. Also, you will provide skilled nursing care to patients in their homes, often under the direction of a Registered Nurse or physician.
